Young At Heart Diabetes Self- Management Education

The Diabetes Self- Management Education program is provided by the

Young At Heart Senior Center and is open to persons 18 and older

who have a diagnosis of Type 1 or Type 2 diabetes.

Patients with diabetes and their family members are taught

to manage diabetes with information on nutrition, exercise, medications,

behavior change and the avoidence of acute and chronic complications.

The classes are taught by registered nurses, dietitians, pharmacist

and certified diabetes educator.
